1. Use the standard protocol/amendment format, which can be found on the AFRIMS Intranet IACUC site to prepare the protocol/amendment. Please fill in all sections (Use N/A when appropriate)

2. Consult with Safety (CPT Melendez), Vet Med (MAJ Phinney), Your Dept Chief, Ms. Somporn (Biostatistician) and all co/associate investigators BEFORE and DURING protocol preparation

3. Obtain all signatures requested on the signature sheet of the protocol. For amendments please send through your Dept Chief and the Attending Veterinarian, MAJ Phinney. If there is safety or statistical issues please have the appropriate person review the amendment before submission.

4. To submit the protocol/amendment for IACUC review

(1) Send an e-version to khun Siriwan (IACUC Administrator) via e-mail at Siriwank@afrims.org AND

(2) Send a hard copy of the document with all original signatures to khun Siriwan (IACUC Administrator) in the Department of Veterinary Medicine

Please submit new amendments/protocols by 4 March 2005 for review at the March IACUC meeting.
